Chaos;Head (カオスヘッド, Kaosu Heddo), stylised ChäoS;HEAd, is the first game in the series, originally released for Microsoft Windows in April 2008. It is described as a "Delusional Science Adventure" (妄想科学ADV, Mōsō Kagaku Adobenchā). Set in the year 2009, the story follows a shut-in named Takumi Nishijou, who finds himself involved in a series of serial murders.
Chaos;Head NoAH received an HD remaster release for the Nintendo Switch on February 24, 2022. It was bundled alongside its thematic sequel, Chaos;Child in the Chaos;Head NoAH/Chaos;Child Double Pack.
An English localization of ChäoS;HEAd NoAH was released for the Nintendo Switch and Steam on October 7, 2022. The Steam release was initially cancelled due to content concerns on the platform, but this decision was later reversed after an outpouring of fan support on social media. It was additionally released for Microsoft Windows on the GOG storefront on October 31, 2022.
